Cancellation Policy

I understand that I must cancel or reschedule my appointment at least 24 hours in advance. Failure to do so may result in a $50 fee charged to the card on file.

Late Arrival Policy

I understand that if I arrive 15 minutes or more past my scheduled appointment time, I may be required to reschedule and will incur a $50 fee.

Appointment Confirmation

I understand that if I do not confirm my appointment at least 24 hours in advance, my appointment may be automatically cancelled.

No-Show Policy

I understand that failure to show up for a scheduled appointment without prior notice (a “no-show”) will result in a $50 fee charged to the card on file.

New Patient Policy

I understand that all new patients are required to have a valid credit or debit card on file prior to scheduling their first appointment.
